My best guess is '[The] third wheel', because the woman behind the couple engrossed in conversation looks as if she's miffed at not being included, and there's a ship's wheel placed prominently above the two women. Or am I barking up the wrong tree (as per a previous challenge!)

The wax seal (?) on the envelope in front of the couple, and their secretive huddle, makes me think 'My lips are sealed"
